Creating a form begins with preparing a document template in which all the necessary fields will be placed. To do this, run the Microsoft Word program and create a new document by clicking on the New command in the File menu (in Word 2003) or by clicking on the Office button and selecting New (in Word 2007). The next step is to display the Forms toolbar. In Word 2003, right-click on any toolbar and select Forms from the list that appears. In Word 2007, click the Office button, select Word Options, and in the window that appears, in the Popular section, check the box Show Developer tab on the ribbon. After clicking OK in the main program window, the Developer tab will appear, which contains all the necessary elements.

After placing all the necessary fields, you need to protect the form from changes. In Word 2003, this is done by clicking the Protect Form button on the Forms toolbar. In Word 2007, click the Protect Document button on the Developer tab and select Restrict Formatting and Editing. In the task pane that appears, check the Allow only the specified method to edit the document and select the Enter data in form fields item from the drop-down list. Save the created form as a template (File – Save As – Document Template (* .dot) in Word 2003 or Office – Save As – Word Template in Word 2007). When opening a template, a new document will be created, in which it will be possible to make changes only to the form fields.
